Yes. Testosterone is a Schedule III controlled substance and requires a valid prescriber-patient relationship, which we establish via telehealth evaluation. We order labs first, review results with you, and prescribe TRT only when clinically appropriate. Prescriptions are sent electronically to your pharmacy or directly to a compounding pharmacy depending on the formulation. Note: Maryland in-person exam may be required for the initial prescription for certain TRT delivery methods — your provider will advise.
Most "Low T clinics" are single-purpose subscription services focused on getting you onto TRT as quickly as possible. Icon Medicine is a full-service family medicine practice. We provide TRT when appropriate based on clinical evidence, but we also address the whole picture — your cardiovascular health, metabolic status, mental health, and lifestyle. We look for root causes, not just treat numbers. And we accept insurance, which most Low T clinics do not.
The physician consultation and workup for ED are typically covered by insurance. The ED medications themselves (sildenafil, tadalafil) may or may not be covered depending on your plan and the underlying diagnosis. TRT can reduce natural testosterone production and fertility (important to discuss if fathering children is a consideration), raise hematocrit (thickening the blood), affect PSA levels, and in some cases increase estradiol. These risks are manageable with proper monitoring — which is why we check labs regularly. TRT is not appropriate for men with prostate cancer, polycythemia, or certain cardiovascular conditions. Your provider will review all risks and contraindications at your evaluation.

This is very common. Standard lab reference ranges for testosterone are wide — a value of 300 ng/dL is technically "normal" but many men feel significantly symptomatic at that level. We look at your total testosterone, free testosterone, SHBG, and symptoms together to make a clinical determination. We also rule out other causes of fatigue and low libido — thyroid, sleep apnea, depression, metabolic issues — to get you a complete answer, not just a number.
