Your Medical Home — All in One Place

Primary care is the foundation of your health. Your primary care provider manages preventive care, coordinates specialist referrals, treats acute illnesses, and manages chronic conditions over time. Having a consistent primary care relationship means your provider knows your full health picture — not just today's problem.

Research published in the New England Journal of Medicine found that patients with a primary care relationship have 19% lower mortality than those without one. The American Academy of Family Physicians recommends annual wellness visits for all adults, and more frequent follow-ups for those managing chronic conditions.

At Icon Medicine, primary care is delivered via telehealth for patients in Maryland, Virginia, and Florida — and in-person at our Hyattsville, MD office for patients who prefer it. Same-day appointments are available daily.

Your Conditions, Explained

Understanding your condition is the first step toward managing it. Click any condition below for detailed information on symptoms, how we treat it, and what to expect.

Hypertension is diagnosed when blood pressure consistently reads 130/80 mmHg or higher. It's often called the "silent killer" because most people have no symptoms until the pressure has been elevated for years. Left unmanaged, it causes heart attacks, strokes, kidney disease, and vision loss.

⚠ Symptoms to Watch For
  • Most people have no symptoms — get checked regularly
  • Severe headaches (especially morning)
  • Nosebleeds (hypertensive crisis)
  • Blurred vision or eye changes
  • Shortness of breath with activity
  • Chest tightness or palpitations
🏥 How Icon Medicine Treats It
  • Baseline labs: BMP (potassium, creatinine), fasting lipids, urinalysis
  • Home blood pressure monitoring guidance (correct cuff technique)
  • Lifestyle counseling: DASH diet, sodium restriction, exercise
  • Medication initiation and titration
  • Target: BP < 130/80 mmHg per AHA/ACC guidelines
  • Follow-up every 4–8 weeks until controlled, then every 3–6 months
💊 Common Medications
Lisinopril (ACE inhibitor) Losartan (ARB) Amlodipine (calcium channel blocker) Metoprolol (beta blocker) Chlorthalidone (diuretic) HCTZ (thiazide diuretic)
📋 What to Expect at Your Visit
  • Review of home blood pressure log (download a free tracker)
  • Lab review for kidney function and electrolytes
  • Discussion of medication tolerability and side effects
  • Dietary and lifestyle coaching
  • Cardiovascular risk score calculation
  • Referral to cardiologist if resistant hypertension

Emergency: If your blood pressure reads above 180/120, call 911 or go to the ER — this is a hypertensive emergency.

Type 2 diabetes is a chronic condition where your body doesn't use insulin effectively, causing blood sugar levels to stay too high. Pre-diabetes (HbA1c 5.7–6.4%) is an opportunity to reverse course. Diabetes is diagnosed at HbA1c ≥ 6.5% or fasting glucose ≥ 126 mg/dL.

⚠ Symptoms to Watch For
  • Increased thirst and frequent urination
  • Unexplained fatigue and weakness
  • Blurred vision (glucose affects lens)
  • Slow-healing wounds or frequent infections
  • Tingling or numbness in feet/hands
  • Pre-diabetes often has no symptoms at all
🏥 How Icon Medicine Treats It
  • HbA1c every 3 months if uncontrolled, every 6 months when stable
  • Annual labs: lipids, kidney function, urine albumin (diabetic nephropathy screening)
  • Annual diabetic eye exam referral and foot exam
  • Nutritional counseling: low-glycemic diet, carbohydrate management
  • GLP-1 agonist therapy for diabetes + weight management goals

  • Review of home glucose log (if monitoring)
  • Foot symptom screening (neuropathy)
  • Medication adjustment based on most recent labs
  • Statin therapy discussion (most diabetics qualify)

The thyroid gland controls metabolism, energy, mood, and dozens of body functions. Hypothyroidism (underactive thyroid) affects 1 in 8 women over a lifetime. Hyperthyroidism (overactive thyroid) is less common but can cause serious heart rhythm issues if untreated.

⚠ Hypothyroid Symptoms
  • Fatigue and sluggishness
  • Weight gain despite no diet change
  • Cold intolerance (always feeling cold)
  • Constipation, dry skin, hair thinning
  • Depression, brain fog, memory issues
  • Heavy or irregular menstrual periods
⚠ Hyperthyroid Symptoms
  • Rapid heartbeat or palpitations
  • Unexplained weight loss
  • Heat intolerance, excessive sweating
  • Anxiety, irritability, tremors
  • Frequent bowel movements

📋 What to Expect at Your Visit
  • TSH and free T4 review; titration of levothyroxine if needed
  • Dosing: take levothyroxine on empty stomach, 30–60 min before eating
  • Symptom review: energy, weight, mood, hair changes
  • Goal TSH: 0.5–2.5 for most patients (lower for younger patients)
  • Endocrinology referral for complex or newly diagnosed hyperthyroidism

High cholesterol has no symptoms — most people discover it on a routine blood test. Elevated LDL ("bad" cholesterol) contributes to plaque buildup in arteries, increasing heart attack and stroke risk. The American Heart Association recommends a fasting lipid panel every 4–6 years for adults with no risk factors, and more frequently with risk factors.

⚠ Risk Factors & Warning Signs
  • High cholesterol is asymptomatic — only found on labs
  • Family history of high cholesterol or early heart disease
  • Diabetes (significantly raises cardiovascular risk)
  • Smoking, sedentary lifestyle, high saturated fat diet
  • High LDL > 130 mg/dL (concern); > 190 mg/dL (high risk)
  • Low HDL < 40 mg/dL (men), < 50 mg/dL (women)
🏥 How Icon Medicine Treats It

  • Annual fasting lipid panel (more frequent if on statin therapy)
  • Dietary guidance: reduce saturated fats, increase fiber, omega-3 fatty acids
  • Statin therapy initiation based on AHA/ACC guidelines
  • Liver function test (ALT) at baseline and 3 months post-statin start

How Primary Care Works at Icon Medicine

Whether you're establishing care for the first time or managing ongoing chronic conditions, here's what the primary care process looks like with us.

1
New Patient Visit

Comprehensive New Patient Evaluation (60 min)

Your first visit is a complete health assessment. We review your full medical history, current medications, family history, and recent labs. We discuss your health goals, perform a telehealth physical examination, and identify gaps in preventive care. Maryland patients may also schedule an in-person physical at our Hyattsville office.

2
Lab Work & Screenings Ordered

Complete Lab Panel & Diagnostic Workup

After your evaluation, we order a comprehensive lab panel tailored to your needs — CBC, CMP, lipid panel, HbA1c (if indicated), TSH, vitamin D, and any other clinically relevant tests. Lab orders are sent to Quest Diagnostics or LabCorp locations near you. Results are reviewed by your provider within 2–3 business days with a follow-up message explaining your findings.

3
Personalized Care Plan

Your Individualized Treatment Plan

Based on your evaluation and lab results, your provider builds a personalized care plan. This includes medication management (new prescriptions or adjustments to existing ones), lifestyle interventions (diet, exercise, sleep recommendations), specialist referrals when needed, and a clear schedule for follow-up visits.

4
Ongoing Management

Regular Follow-Up & Medication Optimization

Chronic conditions require ongoing monitoring. We schedule follow-ups based on your needs — every 3 months for actively managed conditions, 6 months for well-controlled conditions, and annually for healthy patients. Medication adjustments are made based on your response and lab values. You can also message your provider between visits.

5
Preventive Care

Annual Wellness & Preventive Screenings

We proactively track age-appropriate screenings — colonoscopy (starting at 45), mammograms for women 40+, PSA screening for men 50+, bone density scans, and cardiovascular risk assessments. We also keep your vaccines up to date and review your lifestyle risk factors annually to prevent disease before it starts.

Realistic Expectations for Primary Care

Your First Visit

Plan for 45–60 minutes. Your provider will review your complete medical history, current medications, and health goals. Be ready to share past diagnoses, surgeries, and family medical history. After the visit, lab orders are sent within 24 hours and prescriptions to your pharmacy by end of day.

Ongoing Care Schedule

  • Healthy adults: annual wellness visit
  • Hypertension/diabetes: every 3–6 months
  • Active medication changes: every 4–8 weeks until stable
  • Lab monitoring: per condition (A1C every 3 months if uncontrolled)

Lab Work & Testing

All lab orders go to Quest Diagnostics or LabCorp locations near you. Most routine labs are covered by insurance (CBC, CMP, lipids, A1C, TSH). Results are reviewed by your provider, and you'll receive a message with your results and explanation within 2–3 business days. Critical values are addressed same day.

Telehealth vs. In-Person

Telehealth visits are available for patients in Maryland, Virginia, and Florida. In-person visits at our Hyattsville, MD office are available for Maryland patients. In-person is recommended for physical examinations, certain procedures (ear lavage, wound care), and when physical assessment is clinically necessary.

Frequently Asked Questions

A primary care visit includes a review of your medical history, current medications, and chief complaint. Your provider will perform a clinically appropriate examination (visual assessment and history-taking via telehealth; full physical for in-person visits), order relevant lab work or diagnostics, adjust medications if needed, address preventive care gaps, and provide a follow-up plan. New patient visits run 45–60 minutes; follow-up visits typically 20–30 minutes.
Yes. After your telehealth visit, we send lab orders to a Quest Diagnostics or LabCorp location near you. You go in at your convenience (no appointment needed for routine labs at most locations). Results are sent to us electronically within 1–3 business days. Your provider reviews all results and sends you a message explaining your numbers and any recommended follow-up. Critical or abnormal results are addressed the same day.
Yes — chronic disease management is a core focus of our primary care practice. We manage hypertension (prescribing and adjusting medications like lisinopril, amlodipine, metoprolol), diabetes (metformin, GLP-1 agonists, insulin), high cholesterol (statin therapy, dietary counseling), thyroid conditions (levothyroxine management, TSH monitoring), and other chronic conditions. We build ongoing care plans with regular follow-ups and lab monitoring to keep your conditions well-controlled.
For most primary care needs — medication management, chronic disease follow-ups, lab review, sick visits for minor illnesses, and preventive care counseling — telehealth is just as effective as in-person care. The key difference is physical examination capabilities. Telehealth visits use video, so your provider can see you and perform a visual assessment, but cannot perform a hands-on physical exam. For Maryland patients who need an annual physical with hands-on exam, ear or skin procedures, or anything requiring direct physical assessment, in-person visits at our Hyattsville office are available.
We accept most major insurance plans including Blue Cross Blue Shield, Aetna, UnitedHealthcare, Cigna, Humana, Medicare (Original and Medicare Advantage plans), Medicaid and Medicaid MCOs (Maryland, Virginia, Florida), and Tricare. We also offer self-pay options starting at $150/visit for patients without insurance. Call us at (240) 966-4266 to verify your specific plan before your first visit.
Yes. Same-day telehealth appointments are available daily for both new and established patients. New patient slots (45–60 minutes) are available most days. If you're calling for an acute illness or urgent concern, we prioritize getting you seen the same day. Book online at the link above or call (240) 966-4266 to check today's availability.
Yes — medication refill management is one of the most common reasons patients visit us. We can refill non-controlled medications (blood pressure medications, thyroid medications, diabetes medications, cholesterol medications, antidepressants, and more) after a clinical evaluation. We do not refill controlled substances (opioids, benzodiazepines, stimulants) without an established relationship and appropriate monitoring in place. To ensure safe refills, we may request recent lab work before renewing certain medications — for example, we need a recent HbA1c before refilling diabetes medications, and a recent TSH before adjusting levothyroxine.
We coordinate specialist referrals on your behalf. For common referrals — cardiology (for arrhythmia, resistant hypertension), endocrinology (complex diabetes, thyroid nodules), gastroenterology (colonoscopy referrals), dermatology, orthopedics, and others — we send the referral directly to the specialist's office and provide you with their contact information. Most insurance plans require a primary care referral for specialist visits; we handle this automatically so your insurance covers it. We also communicate with specialists after your visit to maintain coordinated care.
Frequency depends on how well your conditions are controlled. General guidelines:

Every 3 months: Uncontrolled diabetes (HbA1c > 8%), blood pressure readings consistently above goal, recent medication changes needing monitoring.

Every 6 months: Well-controlled diabetes (HbA1c 7–8%), stable blood pressure on medications, thyroid doses recently adjusted.

Annually: Healthy adults with no chronic conditions, well-controlled conditions on stable medications for >12 months.

As needed: Acute illness, medication concerns, or any new symptoms. We're available same-day for sick visits even for established chronic disease patients.
Yes — and this is one of the strongest reasons to have primary care that integrates with other specialties. Depression and anxiety frequently co-occur with chronic conditions like diabetes, thyroid disease, and hypertension. We screen for both at every annual visit and can prescribe antidepressants (SSRIs/SNRIs) or refer to our mental health team for therapy and psychiatric care. Weight management is also interconnected with primary care — metabolic conditions like diabetes and hypertension often improve significantly with weight loss. We can initiate GLP-1 therapy (semaglutide) and coordinate with our weight management team for comprehensive metabolic care. See our Mental Health and Weight Management pages for dedicated programs.
